The New York City Community Health Survey (CHS) is a telephone survey conducted by the NYC Health Department.
The NYC Health Department uses the survey results to address health problems and improve services for residents in NYC.
The survey uses a sample of randomly selected adults age 18 and older from all five boroughs.
Survey participants will be asked questions about their:
- Health
- Health practices
- Access to health care
